A Chronic Condition Special Needs Plan, or C-SNP, is a type of Medicare Advantage plan designed for people who have certain qualifying chronic health conditions. These plans may include benefits and care support that are specific to the health needs of people with those conditions.

C-SNP plans are only available to people who meet specific medical eligibility requirements.

To enroll, your qualifying condition must be verified by your physician.
